1.Print the application. (PDF File)
2. Fill out all questions in the application
thoroughly. Pay special attention to details,
spelling and grammar.
Please avoid short simple answers to the
questions. Your profile will be read by potential
recipients who want to know much about who
you are as a person.
3. Sign all required pages
4. Mail the application along with a copy of your
driver's license (or legal photo ID), and 3-5
reasonably attractive, close ups of your face.
(They will be returned.)
EMAILED OR FAXED PROFILES ARE
PREFERRED.
Good photo examples:
...Head shots
...Candid close ups (frontal and profile view if
possible)
...Family photos that include you.
...Any photo where your facial features are
completely distinguishable.
...Some full body photos are helpful.
Do not send:
...Risque or poses sexual in nature.
(respectable photos, please.)
...Laser copies
...Pictures of your children, unless you are clearly
in the photo.
...Distant or unclear photos of yourself
You will be notified if I need additional family
photos.
5. If you believe you qualify for Exceptional Donor
Status, please send with your application one or
more of the following:
...Copies of College transcripts (unofficial okay)
...SAT/ACT scores
...IQ test results, etc
If you need time to acquire this documentation,
send the application and pictures first, and mail
other documents when you receive them.
